Public Health Solutions, CAIPA, and Hyphen Partner to Advance Integration of Social and Medical Care Across New York
Building on its role as an interoperability and analytics partner for Public Health Solutions (PHS) under New York’s 1115 waiver, Hyphen now connects over 300 CAIPA providers to PHS’ WholeYouNYC network
HARTFORD, CT — A new data-sharing initiative is connecting clinical care and social services for thousands of Medicaid patients across New York. Hyphen, a digital health platform specializing in interoperability has enabled the Coalition of Asian-American IPA (CAIPA) to expand screenings for health-related social needs (HRSNs) using Public Health Solutions’ (PHS) WholeYouNYC network. This effort aims to link more Medicaid participants to food, housing, and other essential social services through more efficient coordination between medical providers and community partners.
Providers often face challenges integrating social determinants of health (SDOH) into patient care because screening tools are generally disconnected from clinical workflows, often lack multilingual options, and largely neglect addressing cultural barriers of patient populations. Hyphen streamlines this process by embedding HRSN screenings directly into CAIPA providers’ electronic health record (EHR) workflows.
Through this initiative, more than 300 CAIPA-affiliated providers, including adult primary care physicians and pediatricians, can now screen patients for HRSNs in English, Chinese, Bengali, and Korean. Hyphen surfaced insights that led PHS to expand WholeYouNYC access across the CAIPA network.
