Public Health Solutions Selects Hyphen as Interoperability, Data Partner for 1115 Waiver and Beyond
PHS to leverage Hyphen’s deep experience in addressing health-related social needs to succeed in 1115 Medicaid waiver and other value-based payment models
NEW YORK — Hyphen, the technology company creating connected-care, was selected to be the interoperability platform and data warehouse partner for Public Health Solutions (PHS). Hyphen’s FHIR-enabled interoperability platform will support PHS in managing more than 3 million individuals served by WholeYouNYC, their social care network chosen to participate in New York’s Medicaid Section1115 Waiver.
Hyphen will enable data exchange between PHS and its stakeholders, integrating social care and health care, a foundational principle guiding New York State’s Health Equity Reform (NYHER) initiative. Hyphen’s platform will aggregate all inbound data sources into a population management dashboard and an analytics tool, enabling PHS to meet the state’s Medicaid Section 1115 Waiver reporting requirements, effectively evaluate care delivery pathways, and measure outcomes and performance in current and future value-based payment models.

Hyphen will also establish bi-directional data exchange with Unite Us, PHS’s partner for HRSN screenings, closed-loop referrals, enhanced services tracking and billing with CBOs. Additionally, Hyphen will enable file and real-time API connections with provider health systems and MCOs with whom PHS contracts. Hyphen will also ingest data from Healthix, the health information exchange (HIE) entity serving New York City and Long Island. This will provide PHS with updates whenever members have received social needs screenings or other assessments, or if they have received referrals for social services outside of their existing network.
Hyphen already has deep experience connecting health and social care in New York and in the 1115 waiver programs, supporting 11 of the 17 MassHealth ACOs under Massachusetts’ 1115 Medicaid waiver.
